Hospital Workers ignore dying woman in the ER. Is this an isolated case or a bigger problem with ER care?

News channels are buzzing with the case of a dying woman who was ignored by hospital workers in a hospital ER as she vomited up blood. Her loved ones actually called 911 from the emergency room to try and get her help, but the woman died in the waiting room. Was this an isolated case of negligence or is this indicative of the state of ER care in the US today?
